Interest in under-eye fat repositioning surgery has been growing recently.
As the procedure gains popularity, many misconceptions have also emerged.
Today’s discussion will focus on the effectiveness of lower eyelid fat repositioning and the differences between eyes that are suitable for the procedure and those that are not.
Q. Are there eyes for which lower eyelid fat repositioning is effective and others for which it is not?
Lower eyelid fat repositioning is not effective when wrinkle removal is required.
The procedure is effective when the lower eyelid is puffy while wrinkles are minimal.
When the lower eyelid appears full and makes the face look older, repositioning of the fat can produce a clear rejuvenating effect. In many such cases, the hidden under-eye fat becomes visible, creating a softer and more youthful impression.
This improvement alone can lead to noticeable aesthetic enhancement.
When wrinkles need to be corrected, however, lower eyelid fat repositioning does not provide benefit.
In some cases, wrinkles may appear deeper after surgery. This occurs because wrinkles that were partially hidden by the protruding fat become more noticeable once the fat is repositioned or removed.
Therefore, it is important to select a surgical method appropriate for the specific anatomical condition of the eyes.

Q. What is the difference between lower blepharoplasty and lower eyelid fat repositioning?
When there are many wrinkles under the eyes, lower blepharoplasty is recommended.
Both lower blepharoplasty and under-eye fat repositioning address the skin and muscle beneath the eyes, but their purposes and surgical methods differ.
Lower blepharoplasty is performed when the lower eyelid skin has lost elasticity or when wrinkles and bulging appear due to skin laxity.
Under-eye fat repositioning, on the other hand, is recommended when there is minimal skin sagging or wrinkling but noticeable protrusion of fat under the eyes.
In under-eye fat repositioning, the skin is not incised.
Instead, an incision is made inside the conjunctiva to remove a portion of the protruding fat and reposition it to the hollow area.
Although often compared with lower blepharoplasty, this procedure is relatively simpler and has a shorter recovery period.
Because there is no external skin incision, visible scarring does not occur.
