One of the questions I hear most frequently during consultations is,
“Isn’t under eye fat simply removed?”

Under eye fat removal and under eye fat repositioning are distinctly different procedures.

For some patients, fat removal alone may be sufficient depending on their individual under eye anatomy. However, removing fat without considering the underlying structure may deepen the shadow beneath the eyes and make the face appear even more tired.

Therefore, to achieve a natural result without regret after under eye fat surgery, it is important to select the method that is appropriate for the patient’s current condition.

Today, I will explain why under eye fat develops, the difference between under eye fat removal and repositioning, and which patients may benefit from under eye fat repositioning.

 


 

Q. Why does under eye fat develop, and why do dark circles often appear at the same time?

 

Under eye fat does not develop simply because there is too much fat. It is mainly caused by structural changes around the eyes that occur with aging.

As aging progresses, the supporting tissues around the eyes weaken and the fat beneath the eyes gradually protrudes forward. At the same time, the tear trough becomes deeper due to changes in the volume around the eyes.

When upper eye hollowing and lower eye indentation occur together, shadows caused by light become more noticeable. This can make dark circles appear more severe and create an overall tired or older appearance.

This is one of the main reasons why under eye fat and dark circles often worsen together.

 


 

Q. What is the difference between under eye fat removal and under eye fat repositioning?

 

Under eye fat removal is a procedure that directly removes protruding fat beneath the eyes to reduce fullness.

When the lower eyelid indentation is not severe and the primary concern is protruding fat, under eye fat removal alone may provide a relatively satisfactory improvement.

However, if the under eye area already appears hollow, simply removing fat may deepen the shadow beneath the eyes and make the face look even more tired.

In such cases, under eye fat repositioning may be considered to create a smoother contour beneath the eyes.

Under eye fat repositioning involves releasing the retaining structures beneath the eyes, repositioning the protruding fat into the hollow tear trough, and securely fixing it. By reinforcing the weakened tissues around the eyes at the same time, the procedure also helps reduce the possibility of the fat protruding forward again.

As a result, the contour from the lower eyelid to the front cheek can appear more naturally connected, while both hollowing and protrusion are improved to create a softer and more balanced appearance.

Q. Who may benefit from under eye fat repositioning?

 

The appropriate indication for under eye fat repositioning may vary depending on age and the degree of aging.

For patients in their 20s and 30s, the procedure may be particularly effective when protruding under eye fat creates noticeable dark circles or when the face appears tired despite a relatively young age.

For patients in their 40s and 50s, under eye fat repositioning may be considered when skin discoloration makes the face look more tired or when both sunken orbital fat and a deep tear trough are present.

However, in middle aged patients with significantly reduced lower eyelid skin elasticity, under eye fat repositioning alone may have limitations.

In such cases, more satisfactory results may be achieved by removing an appropriate amount of loose skin and fat and selecting a lower blepharoplasty, which vertically lifts the weakened lower eyelid muscles.

Since every patient has a different under eye structure and aging pattern, the surgical plan should be established through a thorough consultation with an experienced medical professional. This is essential for achieving a satisfactory result without regret after under eye fat repositioning.

 


 

Q. Can complications occur after under eye fat removal or repositioning?

 

Under eye fat removal and under eye fat repositioning are considered relatively safe procedures. However, the tissues around the eyes contain many delicate blood vessels and important anatomical structures, so complications may occur if the procedure is performed carelessly.

One of the most important potential complications is bleeding. If sufficient hemostasis is not achieved during surgery or postoperative bleeding occurs for a particular reason, blood may collect within the confined space behind the eye and increase pressure on the optic nerve.

This may lead to visual impairment. Therefore, if severe pain develops or swelling suddenly increases after surgery, it is important to visit the clinic immediately for an accurate examination.

Temporary double vision may also occur if the muscles involved in eye movement are temporarily stimulated during surgery.

In most cases, this improves naturally over time. However, additional treatment may be required if the symptoms persist.

Although these complications are uncommon, their likelihood can be further reduced when the procedure is performed carefully based on a detailed understanding of the anatomy around the eyes.
