Double Eyelid Revision Surgery Can Become Difficult If the First Surgery Was Performed Incorrectly

After the first double eyelid surgery, many patients feel concerned because the double eyelid line is too high, too low, or awkward.

Many people search for terms such as “lowering a double eyelid line” or “removing a double eyelid line” because they are stressed about their results.

Today, I would like to discuss realistic treatment options for correcting an unsatisfactory double eyelid line, especially when the line is excessively high.

 


 

How to Remove an Existing Double Eyelid Line: Non Incisional and Incisional Methods

 

The method for removing a previously created double eyelid line varies depending on the individual condition.

Many patients can improve the issue through non incisional adhesion release, while others may require incisional scar release surgery.

To determine the appropriate revision method, the following three factors should be carefully evaluated.

  1. Height of the double eyelid line: how high the current double eyelid line is

  2. Strength of the double eyelid fixation: how strongly the existing line is fixed

  3. Degree of eyelid drooping or eyelid correction: whether ptosis correction was performed strongly during the first surgery

If the line is only mildly high and the fixation is not severe, improvement may be possible with a relatively simple release technique.

However, in most cases, all three factors must be assessed together, and an incisional approach is often the more stable and satisfying option.

 


 

Cases Where Lowering the Double Eyelid Line May Not Be Possible

 

Release the existing adhesion and perform dissection

Fat repositioning with appropriate ptosis correction

 

Not every case can be corrected by lowering the double eyelid line.

In particular, revision surgery to lower the line may be impossible or very difficult in the following cases.

Severe tissue loss due to repeated surgeries: If a patient has undergone multiple double eyelid revision surgeries, or if a large amount of eyelid skin and muscle tissue has already been damaged or removed, lowering the line can be difficult.

Excessive eyelid correction: If ptosis correction was performed too strongly during the previous surgery, or if the balance of the muscles that open the eyes has been disrupted, lowering the line may be difficult.

In general, the height of the double eyelid line, the degree of fixation, and the extent of eyelid correction must all be evaluated before deciding whether surgery is possible. If the line is too high and the eyelid correction is excessively strong, revision surgery may be very complex or, in some cases, difficult to perform.

 


 

Possible Side Effects to Consider in Double Eyelid Revision Surgery

 

Compared to the first surgery, revision surgery can take longer because the procedure itself is more complex. As a result, swelling and bruising may also last longer.

However, this is not necessarily a sign of a problem. In most cases, it is part of the normal recovery process.

General swelling and bruising usually improve significantly after about 1 to 2 weeks.

Many patients also worry about scarring.

Since revision surgery removes the existing scar and proceeds with correction, excessive or worsening scars are usually not a major concern unless the case is exceptional.

 


 

Changes in the Double Eyelid Line Immediately After Surgery

 

Immediately after surgery, swelling may make the double eyelid appear thicker or higher than expected.

If eyelid correction was performed together, the results may not appear immediately and may gradually settle over time. Recovery patterns vary from person to person.

Within a few days after surgery, swelling usually starts to go down. As the swelling decreases, the double eyelid line gradually stabilizes.

With enough time, the line will settle into a more natural shape, so it is important not to rush the recovery process and to follow the medical guidance carefully.

Ultimately, there are several possible solutions for unsatisfactory double eyelid results depending on the size of the double eyelid, the degree of fixation, and the degree of eyelid correction.

The most important step is to accurately assess your current condition and find the most suitable treatment plan.
