Today, I would like to explain the differences between a brow lift and a forehead lift, two procedures that many patients often find confusing. Rather than simply listing their differences, I will focus on the key distinctions that can help you better understand each procedure.
Q. What Is a Forehead Lift?
A forehead lift is, as the name suggests, a surgical procedure that lifts the forehead skin.
The key point is that some areas of the forehead have very firm attachments between the skin and the underlying bone. For this reason, these attachments must be sufficiently released from the bone to allow the forehead tissue to be lifted effectively.
After sufficient release, the forehead tissue needs to be secured firmly in its new position. Various fixation methods can be used for this purpose.
One commonly used method is to create small holes in the bone and secure the tissue with sutures or to use an Endotine device for fixation.
An Endotine is a medical fixation device composed of a bioabsorbable material. The Endotine is fixed to the bone, and the lifted forehead tissue is secured onto its small tines, allowing the tissue to remain firmly stabilized by the traction created through the lifted tissue.
More recently, some techniques use tunnel based fixation without creating holes in the bone.
After releasing the tissue, small tunnels are created in the bone. Sutures are passed through these tunnels to secure the tissue. Compared with Endotine fixation, this technique has the advantage of avoiding the placement of a foreign material and uses the patient’s own periosteal tissue to provide stable fixation.
Both techniques can be considered safe when performed correctly and can be selected according to the individual patient.
Although Endotine is one of the most commonly used fixation materials in forehead lift surgery, the surgical technique itself can be applied in different ways depending on the patient’s condition.
In some cases, fixation may be performed using sutures alone.
However, because the forehead skin and scalp are relatively thick, there is generally no need for excessive concern about the Endotine being visible from the outside.
Q. What Is a Brow Lift?
A brow lift is similar to a forehead lift in that it lifts sagging tissue upward, but the surgical approach and the area treated are different.
A brow lift involves an incision just below the eyebrow. The skin beneath the eyebrow is removed, and the tissue is lifted and secured in the direction of the eyebrow.
This procedure is particularly effective for improving sagging in the outer portion of the upper eyelid. If necessary, it may also be performed together with procedures such as orbicularis oculi muscle adjustment or fat removal.
In other words, a brow lift is characterized by a more localized approach that selectively improves sagging in areas where correction is needed.
Q. What Is the Difference Between a Forehead Lift and a Brow Lift?
The most significant differences are the extent of correction and the surgical approach.
A forehead lift provides broader improvement by elevating the forehead tissue from above the eyebrows to the scalp.
In contrast, a brow lift removes skin from beneath the eyebrow and lifts the skin below the eyebrow upward. As a result, the tissue above the eyebrow may relatively shift downward.
Depending on the individual, this may create fine lines in the glabellar area between the eyebrows.
Therefore, rather than simply deciding which procedure is better, it is more important to determine where the actual sagging is located and how that sagging is affecting the eye area.
Ultimately, the key is to accurately identify the area where the tissue has actually descended and determine how that sagging is influencing the appearance around the eyes.
Q. Are There Side Effects From Lift Surgery That Involves Incisions and Tissue Elevation?
Brow lift surgery generally has a relatively low risk of side effects.
However, because skin is removed during the procedure, there is a possibility that the eyes may not close completely.
This is a potential complication of any surgery involving skin excision.
A forehead lift involves a wider surgical area than a brow lift, so swelling may be more pronounced. In some cases, patients may also experience symptoms such as a sensation of fluid movement beneath the scalp.
These symptoms are generally part of the recovery process and typically improve gradually over time.
There may also be concerns about postoperative swelling, temporary changes in sensation, or scarring. However, when the procedure is planned based on an accurate understanding of the anatomical structures and performed by an experienced surgeon following sufficient consultation, these risks can be minimized.
Q. Can I Have a Forehead Lift in My 20s or 30s?
Many people assume that a forehead lift is a procedure intended only for middle aged patients, but this is not necessarily the case.
However, forehead lift surgery involves a relatively extensive surgical area and may affect future revision surgery or other procedures.

Therefore, a forehead lift or brow lift should not be considered simply as a lifting procedure. Instead, the treatment plan should take into account the overall structure and relationship between the eyebrows, forehead, and upper eyelids.
