Tired and Droopy Looking Male Eyes Improved into a Refreshed Look: Before and After Male Eyelid Surgery

Recently, interest in appearance among men has increased, and more male patients are considering eyelid surgery to improve their overall image.

Because the eyes play an important role in determining a person’s overall impression, even subtle changes can create a more refined and refreshed appearance.

However, male eyelid surgery requires a different approach from female eyelid surgery, with careful consideration of the thickness of the eyelid skin, the shape of the crease, and the degree of muscle correction. In particular, rather than simply creating a double eyelid crease, it is important to design the eyes by naturally widening both their horizontal and vertical dimensions.

If surgery is performed without sufficient consideration of the individual structure of the eyes, the result may look unnatural, or the patient may remain dissatisfied despite having undergone surgery. In some cases, revision surgery may even be necessary.

Therefore, today we will review an actual male eyelid surgery case and discuss which eye shape and surgical approach can help reduce the likelihood of an unsatisfactory result while creating a natural and refreshed appearance.

 


 

Q. When Is Male Eyelid Surgery Recommended?

 

There are various reasons why male patients seek consultation for eyelid surgery.

Common concerns include:

 

✅ Eyes that appear tired or droopy due to weak eye opening strength

✅ Eyes that look small or heavy because of a short horizontal eye width

✅ Eyes that appear narrow or stuffy due to epicanthal folds

 

When structural or functional characteristics of the eyes create a tired or closed off appearance, male eyelid surgery may be considered to improve these concerns.

 

Male Eyelid Surgery Before: Ptosis and Multiple Eyelid Creases

In this case, the patient had ptosis, which reduced the amount of visible eye opening and created an overall droopy appearance. In addition, multiple eyelid creases had formed, making the space between the eyes and eyebrows appear relatively wide.

The most noticeable feature was the multiple eyelid creases, which were more pronounced on the left side, resulting in asymmetry between the two eyes.

For this reason, non-incisional ptosis correction was performed to improve the eye opening strength. At the same time, epicanthoplasty, lateral canthoplasty, and lower canthoplasty were performed to naturally increase the horizontal eye width and create a more refreshed appearance.

 


 

Q. Why Do Multiple Eyelid Creases Develop?

 

Causes of Multiple Eyelid Creases

 

Multiple eyelid creases can develop for various reasons related to eyelid structure and differences in eye opening strength.

Common causes include ptosis with weak eye opening strength, age related eyelid skin laxity, or insufficient fat volume in the upper eyelid, which can create additional folds above the original eyelid crease.

Structural characteristics of the eyes may also contribute. In patients with protruding eyes, the eyeball projects forward, which can make the eyelid skin more prone to folding and lead to multiple creases.

In this patient, several of these factors were present together, including protruding eyes and relatively weak eye opening strength due to ptosis.

The left eye had relatively weaker eye opening strength than the right, creating multiple creases on both sides, with more pronounced creasing on the left.

Although the patient had relatively thin eyelid skin, the severity of the multiple creases was not excessive. Therefore, rather than using fat grafting or fat repositioning, improvement was possible by adjusting the eyelid structure itself.

Through this approach, unnecessary multiple eyelid creases were corrected while improving the eye opening strength, resulting in a more natural and refreshed male eye appearance.

 


 

Q. What Should Be Prioritized When Designing Male Double Eyelids?

 

5 Months After Male Double Eyelid Surgery

 

In this patient, the primary goal was to improve the tired appearance of the eyes while maintaining a masculine overall impression. Therefore, rather than creating an excessively noticeable double eyelid crease, a natural tapered crease positioned close to the eyelashes was designed to minimize the appearance of an obvious surgical fold.

In terms of horizontal eye width, epicanthal folds remained on the medial side, while the lateral scleral exposure was relatively limited and the distance between the eyes appeared relatively wide. Therefore, canthoplasty was used to improve these proportions.

In addition, because there was sufficient space from the outer corner of the eyes to the orbital rim, lateral and lower canthoplasty could be performed within an appropriate range to increase the horizontal width of the eyes without excessive correction.

As a result, the eyes became horizontally wider without appearing artificial, creating a clearer and more refreshed appearance.

As this case demonstrates, the goal of male double eyelid surgery is generally not to emphasize a distinct crease as in many female double eyelid procedures, but rather to create a natural and refreshed overall impression.

In general, an appropriate design may include a low crease that is not overly prominent when the eyes are open, a refreshed appearance created by increasing the horizontal eye width, and a crease that does not appear excessively deep when the eyes are closed.

 

 


 

Q. What Is the Recovery Period After Male Double Eyelid Surgery?

 

In this case, the patient underwent non-incisional ptosis correction along with epicanthoplasty, lateral canthoplasty, and lower canthoplasty, and showed the following recovery process over time.

 

1 Week After Male Eyelid Surgery

 

At one week after surgery, yellow bruising and swelling were still present, while swelling in the canthoplasty areas was relatively more noticeable. The incision area was also still somewhat red at this stage.

Because the patient originally had asymmetry between the two eyes, the fixation strength and number of sutures were adjusted differently for each side. As a result, some asymmetry could still be observed during the early recovery period.

 

2 Weeks After Male Eyelid Surgery

As the yellow bruising and swelling gradually subsided, the eye shape began to appear more natural. Compared with the first week, the difference in recovery between the two eyes was reduced, and the overall appearance became more balanced.

The redness around the canthoplasty areas also gradually faded, and the early crease that appeared relatively deep when the eyes were closed began to soften.

 

1 Month After Male Eyelid Surgery

 

Most of the yellow bruising and swelling had resolved, with only mild residual swelling remaining. At this stage, the overall eye shape was gradually settling into a more natural appearance.

Approximately 70% of the redness around the canthoplasty area had improved. The crease that initially appeared deep when the eyes were closed due to early fixation had also softened considerably, leaving only a faint line.

 

6 Months After Male Eyelid Surgery

As the eyelids continued to stabilize and recovery progressed sufficiently, the areas that had initially appeared slightly uneven after surgery naturally settled, resulting in a horizontally longer and more refreshed eye shape.

In addition, most of the visible crease line when the eyes were closed had faded, leaving a natural appearance without an obvious surgical look.

As shown through this patient’s recovery, male eyelid surgery gradually settles into a more natural and defined appearance over time.

Recovery after non-incisional ptosis correction is generally relatively fast. However, the rate of recovery may vary depending on the condition of the individual eyelids and the structural characteristics of the eyes. Therefore, it is important to allow sufficient recovery time and monitor the surgical area according to the postoperative instructions provided by the clinic.

 

Before and After Male Non-Incisional Ptosis Correction with Epicanthoplasty, Lateral Canthoplasty, and Lower Canthoplasty

 

In this case, non-incisional ptosis correction together with epicanthoplasty, lateral canthoplasty, and lower canthoplasty improved the previously droopy appearance and created a more refreshed and refined eye shape.
