Why Autologous Rib Cartilage Is Recommended for Revision Rhinoplasty

 

Revision rhinoplasty involves reshaping the nose after its normal anatomical structure has already been altered or damaged by a previous surgery, making it considerably more difficult and complex than primary rhinoplasty.

In particular, the nasal framework often needs to be reinforced while correcting scar tissue and contracture caused by the previous surgery. For this reason, revision rhinoplasty generally requires a stronger and more reliable graft material than primary rhinoplasty.

For these reasons, many experienced rhinoplasty surgeons recommend autologous rib cartilage for revision rhinoplasty.

Q. Why is autologous rib cartilage commonly recommended for revision rhinoplasty?

There are four main reasons why autologous rib cartilage is frequently recommended for revision rhinoplasty.

✅ Strong support and sufficient graft material: Revision rhinoplasty often requires additional structural support and correction of scar tissue caused by previous surgery. Because rib cartilage is firmer than septal or ear cartilage and provides a large amount of usable graft material, it is particularly suitable for complex revision procedures.

✅ High biocompatibility and safety: Because the cartilage is harvested from the patient's own rib, it is 100% autologous tissue. Compared with synthetic implants, the risk of foreign body reaction or rejection is lower.

✅ Excellent strength and shape retention: Rib cartilage is one of the strongest cartilage tissues in the human body. It can be precisely shaped to reconstruct the nasal framework and provides relatively stable long term structural support.

✅ Sufficient amount of available cartilage: A large amount of cartilage can be harvested from the ribs. This makes it possible to secure enough graft material even in complex revision rhinoplasty cases, helping address both functional and aesthetic concerns.

Q. Does using autologous rib cartilage leave a large scar?

 

Because rib cartilage must be harvested from the chest area, a scar may remain at the donor site.

In general, the cartilage is harvested through a small incision of approximately 2 to 3 cm.

Because of this, some patients considering revision rhinoplasty using autologous rib cartilage are concerned about scarring.

However, when the procedure is performed by an experienced surgeon using precise surgical techniques, the scar can be minimized as much as possible.

Autologous rib cartilage also has a low risk of contracture, and because the cartilage generally maintains its shape over time, it provides strong structural support and high stability, making it a highly effective graft material for revision rhinoplasty.

 


 

Q. Where exactly is the scar from rib cartilage harvesting located?

 

The scar is generally located on the chest area over the rib.

The human body has 12 pairs, or 24 individual ribs. The surgeon selects an appropriate rib for the procedure and harvests the cartilage through an incision of approximately 2 to 3 cm.

In female patients, the incision can often be placed along the inframammary fold, depending on the individual patient's anatomy, which may help make the scar less noticeable.

 


 

Q. How painful is autologous rib cartilage harvesting?

Many patients are concerned about pain when they hear that cartilage will be harvested from the rib area.

In practice, harvesting autologous rib cartilage requires an additional surgical procedure, making the overall operation more complex and potentially increasing both surgery time and recovery compared with procedures that do not require rib cartilage harvesting.

For this reason, careful anesthetic management and postoperative pain control are particularly important during autologous rib cartilage rhinoplasty.

With an appropriate level of anesthesia and precise surgical technique, discomfort during the procedure can be adequately controlled. Postoperative pain can also be managed effectively with appropriate medication and follow up care.

Therefore, patients who are particularly concerned about pain should choose a hospital with an established system for safely managing anesthesia and postoperative pain control.

 


 

Q. How long is the recovery period after autologous rib cartilage harvesting?

The general recovery period after rhinoplasty is approximately 7 to 14 days.

In most cases, the nasal splint is removed around 7 days after surgery, and once the major swelling has subsided, patients can usually return to normal daily activities.

However, when autologous rib cartilage is used for rhinoplasty, an additional procedure is required to harvest cartilage from the rib area. As a result, the operation may take longer and recovery may be somewhat slower.

If the surgical area is extensive or several procedures are performed at the same time, swelling around the eyes, cheeks, and nasal area may be more noticeable, which can slightly extend the recovery period.

In addition to visiting the hospital according to the scheduled postoperative follow up appointments, the following precautions are also important.

✅ Sleeping position: For approximately 3 to 4 days after surgery, keeping the head elevated while sleeping can help reduce swelling and support recovery. For about one month after surgery, patients should also avoid sleeping face down or on their side to reduce pressure on the surgical area.

✅ Alcohol and smoking: Alcohol can slow wound healing and increase the risk of bleeding and inflammation. Alcohol and smoking should therefore be avoided for at least one month after surgery.
